Dr. Capuano has been involved with liposuction since
its development in the early 1980s and introduced the
technique to the Rochester area. Various refinements and
the relative safety of the procedure over the years have
made liposuction the most frequently performed and one
of the safest operative procedure by Surgeons certified
by the American Board of Plastic Surgery (Plastic Surgeons).
The original ultrasonic devices have been used since
1988 in Europe to assist in the removal of fat. These
these devices involved the induction of ultrasonic energy
by means of a probe or canula beneath the skin. The
devices required high levels of technology and engineering
and were at first associated with very significant problems.
Such problems as severe scaring, burns, etc. were not
seen with traditional liposuction. Even in the very
best and most experienced hands, significant complications
seemed to occur now and then even after many years of
experience using probe ultrasonic machines.
Because of the potential complications, the ASPRS mandated
(almost) that its members attend a hands on course.
Dr Capuano attended this course in 1997 and Park Ridge
purchased this expensive equipment.

Recently there has been developed what Dr. Capuano
sees as a safe and reliable method of external ultrasonically
assisted liposuction or EUAL. The newest method of UAL
involves the delivery of ultrasonic energy through the
skin at a particular frequency. The technique uses tumescent
fluid with a patented high frequency (ultrasound) to
transmit the energy needed to effect fatty tissue removal.
Tumescent fluid has been routinely used for many years
during liposuction. The delivery of ultrasound for therapy
and diagnostic purposes has been available for many
years as well. Combining the tumescent technique with
a patented ultrasonic frequency and power, allows a
safe ultrasonic treatment of the fat without direct
contact of a probe or canula on the tissue cells. The
newest system combines the tumescent technique, temperature
monitoring, and non- invasive ultrasonic treatment.
Dr. Capuano has evaluated several of the existing
methods of ultrasonic liposuction. He has attended
lectures on ultrasonic assisted lipoplasty (UAL) in
both the US and Canada. He has used many of the currently
available ultrasonic machines during 'hands on' demonstrations
and observed surgery using UAL devices. He has seen
the results of the originator of the probe/canula
technique, Dr. Zocci of Turin, Italy. Dr. Zocci is
credited with development and refinement of the technique.
Dr. Capuano initially decided that the probe canula
method carried a significant potential for problems.
He initially began using the EUL (external ultrasonic
liposuction) equipment - or the Silberg method. This
method has been demonstrated and discussed at plastic
surgical meetings as well as on the national media
by its originator, Dr. Silberg, as well as by Dr.
Ellenbogen, of Santa Monica California.
